We continuously assess and document all aspects of a young person’s daily living and independence, identifying areas where further support or skill building is needed. This enables us to deliver focused interventions that support steady progress.
At C&C Nexus, our core objective is to deliver high quality, responsive services that enable each young person to reach their full potential. We are committed to creating an environment that supports young people emotionally, practically, and socially as they move towards independence and adulthood. To achieve this, we are dedicated to:
We prioritise the development of independent living skills, offering high levels of emotional support and practical guidance in areas such as health, education, employment, budgeting, cooking, and daily living. Our staff work alongside each young person to encourage confidence, decision making, and resilience.
We provide warm, nurturing settings where young people feel safe, comfortable, and proud to call home. Our properties are designed to promote stability and a sense of ownership.
Our homes offer an environment where young people can reflect, learn coping mechanisms, and stabilise their behaviours with support from trusted adults. We encourage reflection and personal growth as key components of development.
We ensure that every young person has access to support 24 hours a day. In our semi independent homes, a trained team member is present at all times. For solo placements or outreach provisions, we provide flexible visiting support alongside 24/7 on call services.
We tailor the level of support to each young person’s needs. These can be adjusted in consultation with the placing authority to meet emerging or complex requirements, including 1:1, 24 hour care for those with learning difficulties, disabilities, or high risk behaviours.
We deliver support aligned with the ASDAN Training Support in Education, Training and Employment (ETE)
We actively encourage and assist young people in accessing education, training opportunities, and employment, recognising that meaningful occupation is key to positive outcomes.
Each young person receives regular key working support, focused on specific areas where extra input is needed. These sessions build confidence, help overcome barriers, and prepare the young person for life beyond supported accommodation.
We help young people with setting up bank accounts, applying for passports, and navigating complex systems such as Universal Credit to ensure they develop the competence to manage personal affairs independently.
At C&C Nexus, we are proud of our strong partnerships across sectors and our commitment to joint commissioning. By pooling ideas, expertise, and resources with our partners, we ensure our offer is robust and agile, particularly for young people with complex needs who require comprehensive, multi agency interventions.
Our approach is inclusive and co productive. We value and actively seek the input of young people, their families, and professional partners in shaping the services we provide. The co production of solutions ensures that our support is relevant, responsive, and reflective of the lived experiences and aspirations of those we serve.
